The following report appears in the records of accidents in a
major manufacturing concern. A new plating machine has been installed and was
being check out for proper operation. During this check it was fond that
bearing on the caustic solution circulating pump were defective and had to be
replaced. The pump was removed, repaired and was being reinstalled. An
electrician was assigned to make the electrical connections on the same pump.
